The Telford After Care Team are based in Wellington and was setup to provide service users with a structured and daily programme of activities and a powerful network of contacts to assist with practical issues such as housing, health needs, benefits and employment opportunities. We work closely with TACT to offer volunteer training and to are developing joint projects together.
